You do not have to disclose your entire medical history to prospective employers.  You do have to disclose if you have restrictions that may prevent you from doing the "essential functions" of a job.
How did you find your last job?  Typically this is the same way that you will find your next job too. 
A drivers license is one of the most important things to have on your application...Not that you will have to drive any employer vehicles, but rather they know that you will make it to work when scheduled and on time.
Veterans...seek out employers and decision makers who are veterans too.  They have a better understanding for what you bring to the table as far as skills that they can use.

Social Security Disability claimants: How should I report my work history? This is not a resume. 

As a vocational expert for ODAR hearings I find that work histories are often embellished vs. reported accurately.
Make sure that you send a thank you card to an employer after they have interviewed you. Email works too!  Regardless,  it is a powerful way to keep your name above the rest of the applicants.

In my experience a basic one to be asked involves the DOT and how the job descriptions and physical demands match given the fact that the source (DOT) is 2 to 3 decades old.
